SUBJECT: Modified Phase-in of CROWNWeb
The purpose
of this memo is to provide the ESRD Medicare Providers in Network 5
with information about the phased implementation of CROWNWeb. We have
been instructed by CMS to inform you of the following:

On February
1, 2009, CMS will launch CROWNWeb to a small, select group of
providers across the country both large- and small-dialysis organizations,
as well as independent dialysis facilities across the country.
CMS will use a phased implementation approach in rolling out CROWNWeb;
the agency will expand implementation at a later date.

Effective
immediately, facilities should resume processing of CMS-2728,
CMS-2746, and Monthly Patient Activity reports.? Send all hardcopy
forms to the Network immediately, including any forms that you
are currently holding. Please note that CMS Forms Compliance has
been suspended for the next six (6) months, so you will not be
penalized for following earlier CMS instructions.

CROWNWeb
training is still on schedule, so if you have registered for one
of the upcoming face-to-face training sessions, please attend.
The implementation
of CROWNWeb and the CROWNWeb training sessions are not being conducted
by the Network. The Network has limited information at this time and
will continue to communicate information to facilities as it becomes
